Benefits for the Entire Family

This plan includes whichever plan each family member qualifies based on their age and periodontal assessment.
The family member with the highest membership fee pays the full fee, and then for every additional family member, they will receive 5% off the rate for the plan they qualify for.
If the family includes 4+ individuals, then the lesser three fee plans will be given 10% off instead of 5%.

Unlimited comprehensive, periodic, and emergency exams
All 2-D imaging services, which include annual radiographs and any additional imaging needed
Oral cancer and periodontal exams
TMJ and occlusal plane evaluations
25% off orthodontics
15% off any restorative services (includes fillings, crowns, bridges, dentures, night guards, implant restorations)
10% off any cosmetic treatments

Pediatric Dental Plan (Under 13)

$399/year

Pediatric Plan Benefits

2 cleanings (prophylaxis) visits
Unlimited comprehensive, periodic, and emergency exams
All 2-D imaging services, which include annual radiographs and any additional imaging needed
TMJ and occlusal plane evaluations
Fluoride included 1x/year
Sealants discounted to 25% off normal fees

Periodontal Maintenance Plan

$699/year

3 periodontal maintenance visits
Unlimited comprehensive, periodic, and emergency exams
All 2-D imaging services, which include annual radiographs and any additional imaging needed
Oral cancer and periodontal exams
TMJ and occlusal plane evaluations
25% off any further periodontal treatments
25% off orthodontics
15% off any restorative services (includes fillings, crowns, bridges, dentures, night guards, implant restorations)
10% off any cosmetic treatments
